4 indicted in overdose death of teen at Super 8

Four adults have been indicted for their roles in the death of a 16-year-old boy who was found unresponsive in a Super 8 hotel last month.

Andrew Frye was found dead at the hotel on Corporate Woods Pkwy on April 6.

The Summit County Medical Examiner determined he died from a heroin overdose. 

Investigators said Andrew's grandmother Brenda, his mother Heather and her friend Jessica Irons were all in the room before he died. 

Heather was indicted by a grand jury on charges of involuntary manslaughter, child endangering and tampering with evidence. Brenda was indicted on charges of involuntary manslaughter and trafficking in heroin. 

Jessica Irons is facing charges of involuntary manslaughter, reckless homicide, tampering with evidence, possession of heroin and contributing to the unruliness of delinquency of a child. 

During the investigation, investigators determined that Brenda's boyfriend, Donald Callaghan, supplied the heroin. 

He was indicted on a possession of heroin charge.
