Ohio State Highway Patrol are investigating a fatal accident in Westfield Township.
The crash happened around 4:30 p.m. at the intersection of U.S. 224 and Westfield Road.
Michael Kachurchak, 67, of Creston, Ohio was driving a 2002 Nissan Pathfinder, when he was stopped at the intersection, heading south. Kevin Cook, 58, was driving a 2008 Dodge Sprinter van, heading west on U.S. 224, when Kachurchak's vehicle entered the intersection.
Cook's vehicle struck Kachurchak's on the left side.
Kachurchak was pronounced dead at the scene.
Cook was transported to Lodi Community Hospital for his injuries.
Both drivers were wearing seatbelts at the time of the crash.
