HOW TO ENTER: Submit person’s complete name, complete address and birthday to Good Morning Cleveland via e-mail to birthdays@newsnet5.com or post card entry to GMC Birthdays, 3001 Euclid Avenue, Cleveland, OH 44115. Birthdays must be received at least two (2) weeks in advance in order to be announced on air. Birthdays will be read on the applicable day on Good Morning Cleveland between 6:50am and 7:00am. During giveaway period, each day (Monday- Friday,) one entry from those received for that day’s birthdays will be drawn at random to win a certificate for a Dairy Queen ice cream cake. Sponsor reserves the right to verify information submitted. Winners may be required to produce valid state issued identification indicating name, address and birthday, to claim prize.

JUDGING AND ODDS: Limit one prize per person, family or household. Odds of winning are dependent upon number birthday entries received for that particular day.

PRIZES: Estimated retail value of prize is between $10-15. There will be no substitutions for prizes except due to unavailability, in which case, a prize of equal or greater value will be awarded at the exclusive discretion of the Sponsor. Prizes are not transferable, assignable or redeemable for cash. All applicable taxes are the sole responsibility of the winner.

NOTIFICATION: Winners will be notified by phone. Winners may be required to sign and return a statement of eligibility and a liability and publicity release. Failure to sign the statement of eligibility and the liability and publicity release within 60 days may result in forfeiture of the prize. If, in the Sponsor’s exclusive judgment, this promotion becomes compromised in any way, Sponsor reserves the right to withdraw the promotion offer, cancel any method of entry, to void any entries submitted fraudulently or select winners from all eligible entries received prior to act that compromised the promotion.
