VALLEY VIEW, Ohio - The tractor-trailer that had its cab go over the Valley View Bridge Tuesday was found to have several violations.

Valley View police said that the Federal Motor Carrier Safety Administration (part of the U.S. Department of Transportation) inspected the trailer that belonged to Gray Container LLC of Cleveland and found the violations.

No other details were immediately released.

Larry Cunningham, 52, died when the cab fell from the westbound lanes of the bridge just before 11 a.m.

A 5 On Your Side investigation discovered Federal Motor Carrier Safety Administration records that revealed Gray Container was inspected 19 times over the last two years. Records show the company's four listed vehicles were placed out of service on 15 of those occasions.

A company spokesperson said the inspections were routine, the repairs minor and Cunningham was described as an excellent driver who did not have any recent citations.

Police said that several witnesses have come forward to give their account of what they saw happen on the bridge. The investigation is ongoing.

The Valley View Bridge was built in 1976.
