NEWSCHANNEL 5’S TEACHER OF THE WEEK
Newschannel 5 will honor one “Teacher of the Week” each week beginning Monday, March 3, 2014, through the end of the school year in June 2014.
Nominate a teacher for “NewsChannel 5’s Teacher of the Week” by completing the information requested in the form below and clicking the submit button. Or, you can send your nomination, in a detailed letter which must include all of the information requested in the form below, to:

To nominate a teacher, you must be a school principal, counselor, a parent of a school aged child or a student with parents’ permission. Nominations will be accepted for teachers of any grade level, from kindergarten through high school or vocational schools, who demonstrates some (but not necessarily all) of the following five qualifications:
1. Persistence in giving personal best
2. An inspiring example to others
3. Educational success
4. Strong contribution to school and/or community (through classroom activities, volunteerism, school clubs, extra-curricular activities, projects, etc.)
5. Love of teaching
Please include a few paragraphs expressing why you think this teacher should be named "NewsChannel 5’s Teacher of the Week." Be sure to use the "5" qualifications listed above as a guideline. Please include your personal experience with the teacher, along with the details of one specific example in which the teacher displayed some action that really inspired you and others. We are looking for insight into what sets this teacher apart from other exceptional teachers in Northeast Ohio.
A panel of judges will make selections on a monthly basis, but all teachers nominated will be eligible throughout the school year as the selection process is ongoing.
Nominators of the teachers selected will be contacted by the "Teacher of the Week" host. Due to the potential number of nominations, the Teacher of the Week panel will not notify the nominators of teachers who are not selected.
